Registration – You may register on our website through Access Pack, through your instructor or supervisor, or through our customer service department.  The registration form requires certain personally identifiable information, which includes, without limitation, your name, institution, city, and a unique individual user identifier and password.

Newsletters – If you want to receive our newsletter, you will be asked to provide certain information so that we can fulfill your request.  We may ask you for certain personally identifiable information which includes, without limitation, your name, email address, postal address, and telephone number. 

Questionnaires and Surveys – From time to time, we may ask you to complete online surveys and opinion polls about customer satisfaction, your habits and interests, and so forth.  These surveys help us understand how to better customize our service to meet your needs.  We do not keep any personally identifiable information about you in connection with your participation in a website survey unless you specifically agree to provide this information.
